Passport Renewal and Last Name Change

I am trying to do a passport renewal as well as ‘last name’ change (maiden to married) at the same time.

Should the documents that I am providing bear my maiden name or married name?

• BLS Order Form - Should I put married name or maiden name?
• Online Application Form – I already submitted the form online with my maiden name. Should I use married or maiden name? if it is married name, can I resubmit it?
• Nationality Verification Form – Should I put married name or maiden name?
• Green Card – I have my maiden name on this. Is this okay?
• Marriage Certificate – I have my maiden name on this. Is this okay?
• Husband’s Passport Copy (for adding spouse name on my passport) – Has my name with maiden initial printed on his passport for the spouse name section. Is this okay?
• Address Copy – I have utility bill which also has my maiden name. Is this okay?
• Notaries (Green Card, Address Proof, Marriage Certificate) – All have my maiden name too. Is this okay?


I am attaching a notarized affidavit for change in name from maiden to married. Would that suffice?
