Today i got my daughter's PIO card from chicago consulate. I got it pretty fast ...I send it on 19th of june by fedex and received today 3rd of july. I may be able to help some if you have any queries

Questions on PIO submission at Chicago CKGS

Hi - Congratulations first on getting the PIO..must be some kind of record time :-)

I am planning to submit the application on Mon and have some questions if you can help me with that.

1. What are the dimensions of the photo needed. Confusion whether its a US Passport size ( 2 x 2) or a stamp size. If its stamp size then what are the dimensions that i am looking at. Also for an infant do you know if its alright to have the eyes closed in the photo (it works for US passport but not sure about the PIO)

2. Do i need to submit only the PIO application form in 2 copies and rest are required as single copy only ?

3. I know you used fedexbut do you know if they accept credit card for payment?

Thanks for the prompt reply. I checked at few places and noone seems to do a stamp size here. The lowest they can go to is a 2 x 2 so i might have to go with that. I am planning to get some extra copies and trim those down to a stamp size as a back up.

Called up the CKGS to confirm the acceptance of the credit card at Chicago facility and they said yes.

I am having difficulty providing the information for Que. 23/24 (a), (b), (c), (d) where it needs information for parents passports, however there are only one text box so how to provide information for both parents? Is it ok to enter the information in this format -> Father - <psprt #> ; Mother - <psprt #> ?

PIO Queries

Hi

We are applying PIO for our 11month old son.It would be of great help if you could answer below mentioned queries for each form respectively.

ADDITIONAL PARTICULARS FORM
1)All the questions which arent applicable, should we leave blank or mention NO or N/A?

2)In the 3rd page under declaration we need to choose option whichever is not applicable. For minors both theoptions arent applicable, which option should we choose Not as a spouse of Indian origin person?


CKGS ORDER FORM
1)If we choose Application for return of documents by FedEx Courier Service, do we still have to fill Service Provider and Return Airway Bill No in page 2? And also we dont have to send return envelope too right?



PIO APPLICATION FORM
1)All the questions(Like Profession/Business Address) which arent applicable, should we leave blank or mention NO or N/A?

2)Question 4 should we answer if its same as question 1 or leave blank or say NO or N/A?

3)Question 14 a) Permanent Address in India. We(Parents) have different addresses in Passports. Can we enter either Mother's or Fathers permanent address matching in the passport?

4)Question 23 b)Both the parents have indian passports do we have to mention both father and mother passport numbers, date of issue and place of issue or either father's or mother's is enough and send photocopies of both?

5)Question 24) both parents are born in india,do we have to mention both parents DOB and POB in 24 b and 24 c or one of us would be enough?

6)Do we have to notarize only the 3rd page of the PIO Application Form in the whole PIO package?

7)For minor the thumb impression needs to be applied in 3 places:One below the photo and 2 under the declaration where there r 2 boxes. Are there any other place else ?

8)We have to send 2 copies of PIO Forms. After notarization and thumb impression is done. Can we just take a copy of the original? Or do we have to sign n take thumb impressions on both the copies?

Supporting Documents
We both parents have valid Visa Status documents but none of us have valid visa stamp. Do we need Valid Visa Stamp too for applying PIO Card?

Most of them were Yes / no questions so i had answered 'no'. Others where they had asked additional details if the answer was yes, i left those blank.

For a minor - "As a spouse of an Indian Origin person" is the option that is not applicable


CKGS ORDER FORM

If you are planning to pay $15 to CKGS and asking them to use the Fedex then you do not need to fill service provider / air bill. Also no need to send any return envelope.

i had the same case. You can enter either of the address and provide the supporting document.

Yes for 23 a, b, c and d i entered details of both parents as father - <Details> ; Mother - <Details>

for 24 b and c , i entered details of both parents as father - <Details> ; Mother - <Details>

Being cautious, i notarized 3rd page of PIO applications (both copies), Declaration of Additional Particular and Declaration of Parental Authorization.


7)For minor the thumb impression needs to be applied in 3 places:One below the photo and 2 under the declaration where there r 2 boxes. Are there any other place else ?

You will need 2 original copies of PIO with both notarized and thumb impressions at all required places. I know this a pain.....

Section I of Order Form (PIO) and money order amount

Hi all,

I am planning to apply for a new PIO card for my minor child. I have a question regrading the courier service:

I am planning to use your " courier / return courier service" and when I enter my information, it says that I need to pay 30.9 USD ($15 for courier to CKGS office, $15 for return courier to my address and $0.9 for some miscellaneous charges) online (through 'COX & KINGS GLOBAL SERVICES 733 Payment Page').

Now after I pay this $30.9, what would be the amount of the money order that I will sending with the documents? Will it be $205 or $220?

And will I be checking the box 'I' (which says: Application for return of documents by FedEx Courier Service) on CKGS Order Form (PIO)? And if I am not required to check box 'I' (as I would be paying $30.9 through your 'value added service'), do I still need to mention my address to which the documents should be returned on page # 2 of Order Form?
