  • PIO: What all documents to be notarized ?

    Hi,

    Do we need to notarized only the 3rd page of PIO application form, OR do we need to notarize any other documents . Pls help me with the ans to this query.

    This is what CKGS informed me when I asked a question about documents

    "We like to inform that the parental authorization form & birth certificate needs to be notarized along with the PIO Application Form"

        PIO questions

        Hi,

        Please help me with below questions

        1)In all the PIO related forms, when signing DATE and place are you using US date format (mm/dd/yyyy) or indian date format(dd/mm/yyyy)? Please Clarify

        2) In the Place, should we mention both city and state or just the city should be sufficient?

        3) In the PIO application form, if any of the fields does not apply to you should we leave it blank or say NO?

        4) For question 24(a) in PIO application form "Whether the applicant or his/her parents or grandparents or great grandparents was born and was permanently resident in India as defined in the Government of India Act, 1935 * at any time {please see instruction (1) }. If so, following details may be indicated:"
        I am on H1 and living in the US..can I write my name in that field or should I write my father's name that is my daugther's grand father's name?

        5) For the question 20(d) in PIO application form, the US passport of my child does not have place of issue, should I leave it blank or should I fill it with the passport center details where we applied for her passport?

          1) In all signature-date fields, I put date in Jul xx, 2014 format.
          2) I put city, state
          3) I left all fields that are not applicable as blank. That's how I did it for my eldest and it worked out. Did the same for my youngest now.
          4) 24(a) I wrote "Yes. Father's birth details below." and provided my details in 24(b)(c) - Since you are an Indian citizen, you don't have to give the grandfather details.
          5) USA

            In the additional particulars form for all the questions like "Has the applicant ever applied for asylum? If yes, specify details:"..since the answer is No. Should I mention NO in the filed or leave them as blank?

                    I just heard from one of my friend that inkless stamp pad used for fingerprint could be rejected..I used inkless and he told me today that it might get rejected.. what did you use inkless or the one with the ink?

                      If you really want to confirm about the stamp pad, you are better off writing to CKGS at [email protected]. They are pretty prompt.

                      I used regular stamp pad.
