  • PIO Application for my son doubts

    I am applying for PIO application for my 2 month old son and have following list of doubts and hoping to get help out here:

    1. How many sets of documents need to be sent for PIO applications?

    2. Does all documents need to be notarized?

    3. In additional particular form what need to be selected for below questions since my son has born in USA
    I am applying for a PIO card (select whichever not applicable)
    (a) As a spouse of an Indian origin person
    (b) Not as a spouse of Indian Origin person

    4. Any help regarding how to make FEDEX sending and returning envelope

    5. In order-form-pio form what need to be enter for below:
    Service Provider & Airway Bill number

    Airway Bill number would be fedex return envelope airway bill number if i am sending return envelope.

    6. I am residing in Virginia and need to apply at Washington DC Embassy and can you confirm the address where to send documents?

    1. How many sets of documents need to be sent for PIO applications?


    A: 2 sets - here is the checklist I made

    Original US passport
    CKGS Order Form - 1 copy
    Additional Particular Form - 1 copy
    PIO Application Form (3 pages) 2 copies
    Copies of pages of US passport 2 copies
    Photos 4 copies
    US Birth certificate copy 2 copies
    Mother and Father passport 2 copies
    US Birth certificate copy 2 copies
    Copies of first three and last two pages of both parents 2 copies
    Proof of address - Utility bill/drivers license - 1 copy
    Visa status of parents - 2 copies
    Address proof in India - 2 copies





    2. Does all documents need to be notarized?

    No, only the last page of the PIO application form - 2 copies

    Your son doesn't have a spouse yet, so select option (b)

    Get fedex legal envelope and put the regular fedex envelope inside (for return), it should fit

    5. In order-form-pio form what need to be enter for below:

        They no where mentioned about the visa status of parents in the documents check list and also the customer care person said no need to send parents visa status documents. Would it be a problem if I did not send them?

          @Krishna38 - Not sure what you applied for, but here is item 15 from Documents Required page for Minor-PIO:

          Visa Status:
          If you are a non-US citizen, then you must provide a copy of one of the following: your Permanent Resident Card (both sides), Employment authorization card which states 'serves as I-512' or copy of page showing H1 / H4, L1 / L2, F1 / F2, J1 / J2 etc. Non-US passport holder parents who are applying for their kids must also provide US visa status. Transfer Notices and Receipt Notices are not accepted.


          Be prepared to send these documents. Good luck!

            Do we need to send full copy of Parent's passport (I mean all pages) or just first 3 and last two photo copies?

              Item-11 from Documents Required page. Your child's birth certificate with your names included and copies of your passport is required as stated below.

              Parents Proof of Indian Origin:
              If you do not have an Indian passport and you are applying for PIO on the basis that your parents and/or grandparents were Indian nationals, then you must enclose the following documents:

              Copy of proof of both parents current nationality and previous nationality.
              Your birth certificate (in which your parent's name is mentioned) is required. If you are applying as the grandchild of an Indian national, then you must submit your birth certificate as well as your parent's birth certificate. All submitted birth certificates must show the parent's names clearly so that it establishes the relationship with your parents and grandparents, AND
              Copies of first three and last two pages of both parent / grandparent's Indian passport, OR
              Copy of both parent / grandparent's domicile certificate issued by the competent authority, OR
              Copy of your parents PIO / OCI cards, OR
              Any other document that provides sufficient evidence of your parent/grandparent's Indian citizenship, such as a notarized copy of school certificate and a land ownership certificate by which eligibility may be reasonably ascertained. The decision of whether to accept this type of document as proof of your parent/grandparent's Indian citizenship will be made by the Consular Officer and will be final in such cases.

                Guys - Revised ololol's checklist. Just follow this.

                CKGS Order Form - 2 copies
                Additional Particular Form - 2 copies
                PIO Application Form (3 pages) 2 copies [third page notarized]
                Parental Authorization Form - 2 copies [notarized]

                Original US passport
                Copies of pages of US passport 2 copies [see NOTE below]
                US Birth certificate copy 2 copies [notarized]
                Photos - 2"x2" 4 copies

                Copies of first three and last two pages of both parents 2 copies (assuming both parents are Indian citizens - otherwise check CKGS website)
                Proof of US address - Utility bill/drivers license - 2 copies
                Visa status of parents - 2 copies
                Proof of India address - 2 copies (not required if address is same as in the parent's passports)


                NOTE for US Passport Copies
                Copy of the information page. This page contains the picture and basic details of the passport.
                Copy of the endorsement/amendment pages. On US passport books that end with page 24, copy pages 22, 23 and 24. On US passport books that end with page 28, copy pages 25, 26 and 27.

                NOTE:
                - I got other forms notarized as I was told by CKGS to do so
                - I sent two copies of everything just to be safe

                  Few more doubts

                  hi,

                  First of all thanks for updated check list. I had few more doubts:

                  1. While sending PIO package by say FEDEX do anywhere we have to mentioned that this is PIO application or else how will they know this package is for PIO application?

                  2. Do we need to paste 2 photos on 2 copies of Application forms or all of 4 photos just need to be send as it is?

                  3. Who should sign CKGS Order Form in case of PIO Minor application? Do we need to have minor hand stamp print or Father can sign that form or both parents required to sign that form?

                    1. In FedEx, addresses have a "company name" and a "person name". I used following address
                    PIO Department (2nd Floor) (person name)

                    2. You have to paste photographs on the PIO forms. I think they need 2 on forms and 2 additional because last time I remember that they returned 2 photos when I sent 2+4. But just to be safe, I sent 4 in addition to 2 on forms.

                    3. It is enough if father signs the form. Child's thumb impression is not required.

                      I went in person, so cannot confirm, but I read that only the signature page needs to be notarized in both the copies. Also US documents (like ID card copies).
