I want to change my last name and address on my Indian passport and take my husbands name and address. I went on BLS International website and tried to follow the suggested procedure. It is pretty confusing and the phone numbers dont work. I have my Green Card, Marriage Certficate and Drivers Licence under my new name but my passport still has my maiden name. My questions are as follows:
- Do I have to add my husband's name to my passport, he is a citizen of the US and I am a green card holder?
- Do I write my maiden name and old address on the BLS form as it requires to write the information on the passport?
- Do I fill separate form to change my name and address?
