Filling out renunciation form

In the renunciation form, there is a question where they ask you to select one of the two options below.

I have been married
I have not been married.

What do you pick if you are divorced? Are they asking if you have been married ever or are they asking are you married now? If it is the former do you then have to submit proof of divorce when filing for renunciation?

"have been" indicates past tense to me, no?
