    Hi,
    I am trying to apply for my husband's entry visa. I am an indian citizen.

    On the CKGS site, one of the document requirements is to fill out the additional particulars form given at this location http://www.in.ckgs.us/resources/pdf/...cular-form.pdf.

    I need some help figuring how to fill out this form for an Entry Visa application.

    The form says
    "I am applying for a PIO card (select whichever not applicable)
    (a) As a spouse of an Indian origin person
    (b) Not as a spouse of Indian origin person."

    1. First of all, I am not applying for a PIO card so do I fill out this section?
    2. Its sounds really moronic that they want you to select whichever is "not applicable"!!!! I am just missing something totally obvious? I dont know how to complete my documentation without this
    3. For the questions earlier in the document where it says "Any previous nationality held" - they have given no option to enter "no". should i just write that in with a pen after printing out?

    Any help would be appreciated!

    Yes, the Additional Partculars Document is a terribly constructed document. They wanted a generic form for the purpose of compiling data and ended up using a PIO form instead that they decided would serve the purpose. It implies their laziness. They leave it up to the applicant to read between their lines.

    You would want to strike out PIO and indicate entry visa.
    Also you may complete or safely ignore (a) and (b), since it does not matter given that this is not an OCI/PIO form.

    So in your case, you'd want it to simply state:

    I am applying for an entry visa.

    You would then write in your date and place, and sign the form.

    For "Any previous nationality held", you would want to write in "no".
