duration of pio card for new born kid born in usa

Apart from applying for PIO card, do we still need to apply for any VISA?
I applied for a PIO card for my son recently and i am awaiting for the card
Somebody was saying that that they give PIO card for only 6-9 months as the facial features of the kid change a lot. Is that true?

PIO card is valid for 15 years along with a valid passport. You need to get the PIO card endorsed whenever the passport is renewed.

For a newborn kid in USA, US issues 5-year passort. After 5 years, when you get your kid's US passport renewed, you need to get the PIO card endorsed with the new passport number.
