Got the PIO Card today and luckily by gods grace everything was smooth
Obtained the card in 14 days

Passport sent by post/courier number 08/13/2014
Your application has been collected from the Embassy/Consulate and is being sorted. Your application will be ready for collection from our Application Centre within one business day. 08/12/2014
Your application has been submitted at The Indian Embassy for further processing. 08/01/2014
Your application is in process. Your tracking id is USA YYYY ZZZ 08/01/2014
Your application has been received and will be submitted subject to verification. 08/01/2014

A - Forms and Payment Details
CKGS order signed by both the parents, including money order number
Additional Particular Form
PIO application form. Notarized the third page of the form.
Parent Authorization Form. (Did not notarize this)
Money order 220 USD (paid 15 USD extra to get the document back on my home address)

B- Son’s Docs
Original US passport
Photocopies of US Passport Copy
Photocopies of Birth Certificate Copy
2 extra photographs

C-Dad's Docs
Photocopies of Passport (first three and last two pages)
Photocopies of US Driving License
Photocopies of H1B approval notice
Photocopies of Visa

D-Mom's Docs
Photocopies of Passport (first three and last two pages)
Photocopies of US Driving License

Yes, god must have certainly handheld your application throughout the process for it to have been this smooth. Unfortunately, I doubt this is an indicator of how the process will be in the future or for everyone.

Can you please tell me from where did you get the 3rd page of the PIO application form Notarized from? I am trying to get it done for my son and when i went to a bank to get the form notarized, they said they will have to attach a separate sheet and notarize on that and not on the PIO form as it doesn't have a legal notary clause. I come under Houston Jurisdiction. Any of you, any help regarding this is appreciated
