OCI fedex prepaid label - SFO office

Can someone help me on how to fill out prepaid label using Fedex
I want to include 2 prepaid fedex return labels to send the OCI and Renuciation back to me.

1. What "from" address should I put in fedex return label? SHould I mention any specific application number here?

2. What I do not understand is COurier service option availalbe under value added services in ckgs website. What is this for? Should I fill up this one too?

Get 2 prepaid envelopes from FedEx.
From Address: CKGS address
To Address: Your address.
