CKGS forms and other PIO documentation questions
Can you please address following queries. I am filing for PIO for my minor child.
1. Do I need to notarize 3rd page of the PIO APPLICATION form?
2. In the CKGS ORDER form, who should sign? Will it be my kid’s thumb impression or parent’s signature?
3. In ADDITIONAL DECLARION form, I understand both the parents must sign if applicant is minor child. Do you need minor child’s thumb impression as well ( along with Parent’s signature)?
4. What is PASSPORT WITHDRAWAL order form? Do we need this if we are filing for minor child’s new PIO?
5. In PARENTAL AUTHORIZATION FOR MINORS, what should we mention in ‘ do hereby authorize my child to leave the United States of America from’ ?
6. Should I Money Order include $15 for FedeX service or this amount will need another MO?
7. In the ‘Documents Required’ Section in CKGS website, what does #17 mean(If the address in India is different than the one in Indian passport, please Provide proof in r/o Indian Address?)
