Urgent help needed !! - Notarization of PIO Application Document

Dear All,

We got our documents & signatures notarized on the following documents for PIO application for our 9 month old son.
• PIO Application form (2 sets)
• Parents authorization form
• Applicant’s birth certificate
• Parent’s passport

We got this notarized by Bank of America and we have a question regarding the same. The notary informed us that they can notarize on the Standard California State Notary form and attach the same to the PIO application form. He can not put his seal/stamp on the 3rd page of the PIO application form itself.


Please let us know if that is valid.
