I want to apply PIO Card for my daughter. We live in Jersey City, NJ and will be applying to New York jurisdiction. I was looking at the Process and had a few questions
1. CKGS Declaration Form, CKGS Disclaimer Form & CKGS Order Form
a. For ‘Signature Of Applicant’ in these forms shall I put my daughter’s thumb impression or should it be signed by me and my wife or signed only by me (father)?
b. Do I need to notarize these forms?
2. My Indian Passport has Old India Address (not the Latest one) but my wife’s Indian Passport has Latest India Address. Do I still need to provide Proof Of New Address? If yes, what shall I provide as Proof Of Address for India?
3. PIO Application Form
a. 23 (a)/(b)/(c)/(d) – Do I need to provide both Parent’s Indian Passport details or Father’s details is fine?
b. 24 (a)/(b)/(c) – Do I need to provide both Parent’s Birth details or Father’s details is fine?
4. Apart from PIO Application Form are there any other documents that I need to notarize?
5. Do I need to send Original copy of Utility Bill or photocopy is fine for Proof Of Residence?
