we are planning to apply for PIO card for my daughter and i have a couple of questions regarding this.
1. I have taken a cashiers check for the amount $220 (including $15 fedex courier service charge for return documents).Now do i need include a self addressed envelope as well?
2.Since my baby is a minor ,I am assuming we can leave her signature columns in CKGS order form,disclaimer form etc blank.(we have already notarized the parents authorization form and took her finger prints in application form.) please correct me if i am wrong.
any quick help is highly appreciated
