PIO: Given Name in Parental Authorization form missing

I need to apply for my Daughters PIO and have submitted the documents to CKGS today by Fedex.
I have notorized the Parental Authorization form and just now noticed that I have not specified the Given Name in the form. I have specified the Name (as given in Passport), however somehow missed the Given name field.

Can you please advice if it Ok to have Given Name field blank? As I said earlier that the Name (as given in passport) is specified and the document is notorized.

Do you know if CKGS would reject the application because the Given Name is missing? Has anyone ever experienced it?

I can. But I'll have to get the document notorized again...
I hope you dint mean adding hand written name in already existing (notorized) document?

If the field is blank currently, you can just add it.

Crossing something and writing again would not be recommended.

Just one last question. I have read on the CKGS site that Hand Written forms are not accepted.

If I add this Handwritten name on the Parental Authorization form, are there any chance of getting it rejected.
