Affidavit for Lost/Damaged and Passport Expired for more than 6 Months
Hi
I am in process to renew my passport but its expired more than 1 year. So it requires affidavit which is mentioned in the title of this post.
But there is confusion about signing on the form. who is depodent in the form is it me or the notary who notarized my copy?
Do I need to sign somewhere in the form as well?
The BLS international helpdesk email or phone are not responsive. I am trying to contact on [email protected] and 1-845-999-0726 but no reply.
It will be great help if someone can clear this confustion for me.
