US Citizen Minor - PIO Card conversion to OCI

Hi, Followed the instruction and filled the form online ..now have pdf file. Couple of questions:

1. I know there is no OCI fee but $45 processing fee but do not see any option to pay the fee. At what stage this fee will be payable.

2. In list of docs..primarily I need to send my daughter US passport, PIO card and parents passport along with pdf file print to NYC office. Anything else I missed out ?

3. Somewhere I noticed that it says parents signature shd be notarized..is this really required or signature can be simply same as passport signatures. If notarized needed..I am not sure who in US notarized such docs.

The processing fee is $20, and add $15 for the return of documents. This should be paid before sending your application, as you have to include payment or proof of payment if done online on the CKGS website. You should download the check list and go through it as it looks like you are missing quite a few things ( documentation required with OCI). The checklist also mentions clearly that for a minor, Part B of the OCI application has to be notarized. Any public notary here will do it.
