PIO endorsing with new Passport number in India.

I am having my PIO endorsed with my new British Passport number
in India.Out of the requisite documents there is a manual application form ?
Since I am applying in India can I use the manual application form available
on immihelp,maybe edit it a bit ?I am asked for photo copies of Passports
old and new do they need to be colour and include even blank pages ?

Any advice much appreciated.

Has anyone upgraded to OCI from PIO in India ?
