Apply for passport with SURNAME change

I am from INDIA , Ahmedabad,

I need to apply for Fresh Passort first time

I have surname(LASTNAME) change from PATIDAR to PATEL in my Docs as following,

PATIDAR VIKAS MANOJBHAI in
1. School Leaving and Degree certificate
2. Birth Certificate


PATEL VIKAS MANOJBHAI in
1. PAN CARD.
2. AADHAR CARD
3. ELECTION CARD


I have done Gazzeted Copy for surname change.

So Can I apply for passport with new SURNAME change.

Please guide what can be done in this scenario.
