OCI card Passport Match Up

Hi All
1)I filling up OCI MATCH UP REQUEST FORM to send passort for MINOR, in the form it asks for applicant's signature, so for Minor's
do we need thumb impression or parents can just sign on behalf?.

2)Also for 'mode of collection' for passport back, do I need to take pre-paid envelope, but I have already paid 30 dollars (15***5) online during initial application submission,
so do i just need to pay 15 dollars for sending the passport?.

3)Also where do I get 'airway bill number'?.

1. Parents can sign.
2. If you already sent money for the return courier, there is no need to send it again. Make sure you mention somewhere that you have already sent the amount.
3. If you already sent money for return courier this doesn't apply in your case.
