Change of spouse surname after marriage - Indian passport...

Hi,
I wanted to check on the procedure to change my spouse's middle name and surname on her Indian passport (i.e. to reflect my own name & surname respectively), we have the marriage certificate. I checked on the BLS website, it lists the procedure only for name change i.e. maiden name to married - > the AFFIDAVIT TO BE SUBMITTED FOR CHANGE OF NAME BY WOMAN APPLICANT CONSEQUENT UPON MARRIAGE.

Do I need to submit the above affidavit, or does this process involve advertising the name change in US & India newspapers. Appreciate any inputs on the same.

My name on passport: P S P
Spouse's current name on passport: S J K --> (J - Spouse's father's maiden name ; K - Spouse's family name before marriage)
Required spouse's name on passport: S P P

Submit the notarized affidavit while applying for passport renewal..

Thanks for the response. Just want to quickly clarify, does it mean I have to fill the affidavit -> http://www.blsindia-usa.com/passport...fNameWoman.pdf and get it notarized. OR the procedure below needs to be done as mentioned on the BLS website:

Change of name (ADDITION OF LAST NAME OR ANY CHANGE IN FIRST OR MIDDLE NAME) is required to be advertised in two daily newspapers, one in USA and another in India (being circulated in the area of applicant’s permanent and present residence in USA).

Name change after marriage

I have a similar question.
I am a Indian citizen female , currently on a H1 Visa working/ living in the US. I am about to start the Green Card process since my marriage to a US citizen 3 months ago. Prior to the GC process I was advised to complete the name change as this will avoid the hassle of making the correction my documents in the future.
I would like to change my middle name and last name (new middle name and last name will be husband's family name)

Questions are:
- would I need to advertise in both India and US newspapers?
- or just fill out the affidavit and notarize it prior to rectifying it on my Indian passport?

Any input on the steps for going about this name change from an 'Indian citizen in the US, on a valid visa married to a US citizen' or from someone who is familiar with this process would help.
