Renunciation form question

I am filling up the online form for renunciation application. I am reviewing the RENUNCIATION form before printing. The form shows as follows

1. I YYYY XX YYYYYYY, am of full age and capacity..........
...
....
5, Father's name : AAAAA BBBBBB

The instructions indicate the names should be as the Indian passport. In the passport the SURNAME is YYYY X and the given Name is YYYYYYY. Similarly SURNAME is AAAAA and given name(s) BBBBB in the father's passport. I would like existing applicants who succesfully applied to tell me if the above is OK
