Naming Convention to follow in PIO/OCI forms

Finally started working on PIO to OCI conversion..

Any idea what is the naming convention to be followed for applicants/parents where ever single line is present?. Example many places single line is present for father name. What is the naming convention to follow?.

"First name, middle name, last name" OR

"last name, first name, middle name" OR

"first name, middle initial, last name".

Last time when I applied for PIO they rejected my application as I used "first name, middle initial, last name" format and I had to send new application with "last name, first name, middle name" format.

the application is asking specifically where to fill the surname and given name. make sure same as your current passport. and parents, spouse name must full name as on current passport

My query was on forms having single line item for name. Passport will have separate lines for last name & given names so no confusion there.

Which is correct from these?

"First name, middle name, last name" OR

"last name, first name, middle name" OR

"first name, middle initial, last name".
