Renewal of passport from USA

Hi there, I wanted to renew my Indian passport in USA. I went to below link:-

here I got link to complete application form, which is :-

when I register myself and login to fill application, Sample Application is different from application I got from above link. User guide lines not matches with the form I got from above link. I got one form with above link which does not matches guide lines mention in checklist at number 3 at link:-

Can you please help me to understand the procedure and what online form I should actually fill.

I heard abt the CKSG site from Immihelp website but somehow the checklist document they show is no more supported as it is linked to BSL.

CKSG has their own form and process.
