OCI registration in Lieu of PIO at Hyderabad

Hi,

I need to convert my kids (7 and 10 years) PIO card to OCI at. They are US citizens but currently residing in India

Couple of questions:

1. In the OCI application form in lieu of PIO, it asks for Place of Issue of the passport. We renewed their passport at US Consulate, Hyderabad. But in US passport there is no field "PLACE OF ISSUE". There is a field "AUTHORITY" and it has US Department of State. My question is, should i put US Department of State in the OCI application?

2. Since the kids are 7 & 10, is it required to upload their signature or thumb impression?

Appreciate quick response as 30th is deadline.

2. Signature should be fine (I had my child sign when she was 6)
