Indian Passport renewal via CKGS, some questiions

I am in the process of renewing my Indian passport through CKGS and I have the following questions

1. Should the NRI Passport application form be notarized? Specifically Q29

2. Should the National Verification Form have a photo attached?

3. Should the National Verification Form be notarized?

4. What is the passport fee amount to be entered in the NRI Passport application form? Total cost - CKGS fee - Shipping?

5. I bought shipping from CKGS, I am able to print label to send my application and documents to CKGS, do I have to send a return envelope so that CKGS uses that when they ship me my new passport, or they'll take care of it?

6. Do I need to send proof of permanent residence in india, notarized?

Please help me with the above questions.

4. What is the passport fee amount to be entered in the NRI Passport application form? Total cost - CKGS fee - Shipping? - I have given it as 75$ which is the passport fee mentioned in receipt.

5. I bought shipping from CKGS, I am able to print label to send my application and documents to CKGS, do I have to send a return envelope so that CKGS uses that when they ship me my new passport, or they'll take care of it? --no need to send a return envelope. ckgs will take care of it.

6. Do I need to send proof of permanent residence in india, notarized? - yes

Please help me with the above questions.

Reference Address proof of Indian residence to be Notarized for renewal of Indian passport in the US -
There is no way I can get my Indian address proof notarized in the US since originals are in India and urgently need to renew Indian passport but with Indian address on it.
Has anyone found a way to get this thing done?
