No surname in Indian passport, but in new US passport

Hello,

My Indian passports have my given name as :TZZZ MZZZ Surname: blank. My US passport has it listed as given name:TZZZ Surname: MZZZ.

1) How do I fill out my OCI forms - do I list my name as it is in the Indian or the American passport?
2) Do I need to check the 'Have you ever changed your name?' column?
3) Do I need to provide additional documentation?

I'd really appreciate help! This is absolutely confusing.
