Passport Renewal in USA

Confused after going through most of the sites regarding the Number of photos to be sent (Shipping) to CKGS.

It says three everywhere.

One to affix in Nationality Verification form.
One to paste in NRI Application form.

And one extra to be sent or

Three more after one each in the above forms. Which would be 5 in total

Updated: Can send 3 photos loosely or Affix where required (One to affix in Nationality Verification form. One to paste in NRI Application form and send the third in shipping pkg)
