OCI Shipping service was missed

Hi,

My situation:

Applying for 11 month old daughter's OCI
Parents and grandparents are born and still Indian citizens
Applying in NY jurisdiction

Complexity:
Couldn't buy shipping service from CKGS, and it selected walk-in for me. I actually want to buy from CKGS. I already made payment so no option to file new application

Questions:
1. How do I change the mode from "walk-in" to shipping with label bought from CKGS?
2. Do I need to send original US passport along with physical application? (not very clear from website), or do I send it later in time?
3. Should my daughter put thumb impression on additional particulars form, or affidavit in lieu of form?

Yes to 2 (send passport with application if they are asking in checklist. Every jurisdiction is different. I sent original passport and got it back in 1 week) and 3. Not sure about 1. It is stupid to ask for thumb impression but that is how it works.
