OCI for minor question

Guys - I've completed both CKGS & Govt website applications for a one year old and just have to send documents into the NYC ckgs. On one hand, they state 'notarized' but on the same page, it says 'self-attested'. Can someone clarify which documents, if any need to be notarized by a notary officer?

Also, a few docs have Applicant signature so I'm guessing put the thumb print in each box. Right?

Got it - thanks. Where do I get the barcode to affix to the mail-in package?

When you are done with the online application, 'My Account' will be visible to you and when clicking on that, there will be a button to print the barcode.
