What documents are required to get Sworn Affidavit from SFO Indian Embassy?

I am currently in California and my Wife and Son are in India. I have to apply for a passport for my son.

I was told that i need to get sworn affidavit from Indian Embassy(in my case SFO)?

My question is what documents are needed to apply for that affidavit?
What all documents are required to be sent to Indian embassy? I am planning to mail the whatever documents are required, so can anyone share their mail experience?

Sworn affidavit not require

You just have to fill out Annexure D form, get it notarized and send that doc to India.
