OCI Form Clarifications - Please Help

My spouse and I are of Indian origin and got U.S itizenship due to Naturalization.
Could anyone please let me know

1. Do I need to fill out separate form for each family member even for kids (minors)?

2. If I opt for "In-person Form Submission", can I go in person and submit all the forms or all the family members must come along for form submission?

3. Do I need Sworn Affidavit? My spouse is Indian origin like me. So, confused.

1. only one application form for each applicant
2. appointment for each application, and submit within the appointment date
3. Yes. I know the form really look silly but no choice
