Affidavit in Lieu of Originals

Doing a OCI renewal for passport update. I see this in the checklist but I don't see any place I can get this form. Where can I get the Affidavit in Lieu of originals?

Also, does notarizing this form mean I do not need the other papers notarized (e.g. US passport, Parental Auth Form?)
