Sample form for Joint affidavit to be given at Indian embassy Chicago

Hi all,
need a help . Have to apply for registration of birth of at Indian Embassy at Chicago. We need to submit a "joint affidavit with the declaration that they have not obtained Passport of any other nationality,". The above affidavit is not present in the Embassy website. Can you please suggest where can I get a sample affidavit for the same. Thanks in advance.
