Is it possible to see/download "Affidavit in Lieu of Originals" form before paying? I would like to see the form so that I can check if I've all the documents mentioned in the affidavit and I can't seem to find the form. I'm at the page where it asks to Pay.

Anyone has idea about this? Or is there any link to sample form?

***, I am unable to find it either. Anyone has that form? Am I going to see that form after I pay?

After payment and govt forms

After you finish payment and govt forms, you will have to sign in again to CKGS. You will go a page with all the documents (except the forms from govt website). You can print the affidavit in lieu of original form there, get it notarized and attach it. You will also get the checklist at the point. It may be weird, but that is how it works!
