Indian Passport Renewal Change in Appearance Affidavit required?

I am applying for renewing my Indian passport at the SF consulate.
At the stage where it gives the Affidavit for Change in Appearance And Signature I selected No and it printed the form saying 'has not changed'. My question is should I still get this notarized and send it with my application even though my appearance and signatures have not changed?
