For sending PIO application by mail to NY, can someone please verify if the following list of documents is correct?
I am sending this for my 3 month old daughter.
First set of documents
1. PIO application form
2. Daughter's original US passport
3. Daughter's US passport photocopy (all pages)
4. Daughter's original birth certificate
5. Daughter's birth certificate photocopy
6. 2 photographs of my daughter (one is pasted on the application form)
7. Father's original Indian passport (I have an expired and a new passport. Do I need to send both?)
8. Father's Indian passport photocopy (all pages)
9. Father's birth certificate photocopy
10. Mother's original Indian passport
11. Mother's Indan passport photocopy (all pages)
12. Mother's birth certificate photocopy
13. Photocopy of 2 documents as proof of current US address (telephone bill and cable bill)
14. One Cashier check for application fee and mail back charges ($155 + $15 = $170).

1. Do we need to notarize all pages of the passport?
2. Do we need to send Father's and mother's original birth certificates?
3. Do we need to write anything on the back of the cashier check?
4. Do we need to send photocopies of our Green Card?
