Changes in passport

Dear Sir,

I have 2 questions noted below :

1. I want to add my wife name in my passport, which documents will be required for this.How much time will it take.

2. I want to make my wife passport, she have only one residence proof RASHAN CARD there is no other residence proof which is mentioned in passport application form. If i will have my wife name in my passport then can that work as a 2nd residence proof.
