Replacing lost passport

Hi,

My passport is lost and want duplicate passport,

I had registered a police complaint, I am filling online form on one section it is asking for file reference no.

I do have the photocopy of my passport, but unable to locate the file reference no. appreciating if you let me know where to locate file reference no in passport,
