Change last name on indian passport

Hi,

I want to change my last name on my indian passport because i got divorced so i want to change my last name to my maiden name and its also expired in june 2007, so what are the requirements for that, so what should i do renewal or new passport, and what about my Permanent resident card should i change my last name on that also and so how many days it will take ?
