Want help regarding applying for a fresh passport

Dear Sir,

I am a permanent resident of Distt. Moga, Punjab (INDIA). My nationality is Indian and i am unmarried. I would like to apply for Fresh Passport.

I would like to know the following questions-

1) Is it compulsory to have Surname for getting Visa for Canada ?

2) Should my Surname must be the same as of my all other family members or it can be different from other family members ?

3) Can i use my nick name as my surname ?

Please guide/help me out. I shall be very thankful to you.
