renewal of minor's passport @ NY indian consulate

We live in NJ. I need to renew my 5-year old Son's "indian" passport at NY Indian consulate.

1) do both parents need to personally visit the consulate or only I (Father) alone can go and apply?

2) If I am going alone, do I still need to take the notarized copy of Affidavit (NOC) signed by my wife?

3) do my Son (the applicant) also need to come to the consulate?
