Query regarding Change of Name in Passport

Hello

I don't have the Sur Name on My passport and while I am applying the US Visa, they are saying that its mandatory to have the Sur Name on the Visa for applying US Visa so can you please advise that whether this is the case and also If I want to include my Sur Name in my Passport so what will be the procedure and free structure and the Time stamp required for the that.

You need to get it changed in the passport by contacting the regional passport office.
