Filling Renunciation form for first time Visa Application

Back of the Renuciation form says:

I, the undersigned, hereby state that I am an citizen otherwise than by naturalisation that I
am householder; and that I am not the solicitor or agent of.......................................... I
vouch for the correctness of the statements made by........................................ in his
application for.............................................

Does this mean I need to find a person who is a US Citizen by Birth? I saw some earlier forum message indicating that it could be signed by anyone other than the applicant. Just wanted to clarify before I send my Visa application. Appreciate anyone telling what they actualy did.

I have the exact same question, until I came here I didn't know that the second page was perhaps supposed to be filled out by a notary public or something..

Why are there no proper instructions with this document and would it hurt to proofread the document..jeez..
