Name change in passport

Hello Sir / Madam ,

I would like to seek your help regarding changes in my passport. I want to go for a plastic surgery for a scar on my face , which is also a mark of identification in my passport. After this surgery this scar will be reduced significantly , but other features of face will remain same. The Scar will be reduced to juat a hair line mark.

Could you please advice me regarding the formalities i need to complete for this purpose.
