Photo question for PIO at Chicago Consulate

Hi:

The application form for the PIO cards says that we need to send 4 photographs, but after reading the detailed instruction I came to know that they need 5 photographs. Which one is correct?

If they need 5 photos, I have already got 4 photos, can the 5 photo be different from the 4 photos
