From Chicago consulate. I stapled a small piece indicating what each document is for. I think that saved some time for them to sort out the documents. Applied on Oct27, received on Nov 5th.

When I was in DC embassy I heard some worst things.
Instead of PIO card I just applied Visa for my son and got it in 1 week. Next year at some point I will apply OCIO instead of PIO as we become eligible for citizens.
