Passport Renewal

Dear Sir?Madam:

I want to apply for renewal of an Indian Passport, expired May 3, 2008. I have the APPLICATION FOR INDIAN PASSPORT form (with Washington DC address). Is it OK to use, I live in Texas.

I want to verify the fee. It is $40 your handling fee, addressed to whom? What is the Consulate fee?
