PIO card Questions

Hi,

My Baby was born in Columbia,SC on 12/23/2008.Me and my wife are indian citizens with valid passport.I have shifted to NJ now.I want to have PIO for baby as soon as possible,because we have to go back to India by 15th Feb.Please let me know answers for following questions.Thanks in advance for answering them

1. Where should I apply for the passport? Should it be Washington as baby was born in SC or it should be NY as I am applying fron NJ.

2. What all documents do I need to send with application form?...Do I need to send original passports or notarized copies will be ok?

3. Do I need to send any document for US residence proof also...I don't have any Residence proof as I will be staying in Hotel right now.

PIO card

I have got the passport done.
My question is as I have just moved from Columbia,SC to NJ where should I apply for the PIO.Also is it mandatory to send current US address proof also as I do not have any address proof in NJ as of now because I have just shifted here.
