Removal of Ex-husband name from Passport

I came to US 2 years back. Before I travelled here, Divorce case has been filed by my Ex-husband. After I came here Divorce has been accepted and right now we are legally separated. Could anyone advice me the procedure to remove the ex-husband’s name from my Passport?

Take the divorce copy and apply for a passport at the nearest consulate. Refer to their website for the form. All visas remain valid.
